3. A flawed process or using the wrong tool for the job. That may sound obvious but processes evolve with the businesses requirements and technology is frequently expected to continue to deliver with little or no modification, until it is entirely inappropriate and nobody noticed

For small businesses especially, each of these a false economy because, whilst no capital expenditure is being made, immeasurable amounts are being lost in lost productivity. Realising this and spending a small amount of money early can improve productivity and therefore earnings.
